| Insulin is the mainstay of treatment for Type I diabetes. It has also been recommended for Type 2 diabetes patients if diet or oral medications are not effective in lowering blood glucose and is often used in conjunction with oral medication. In the past 20 years, a variety of insulin preparations and insulin delivery systems have also been introduced. These products permit treatment schedules to be individualized to meet the needs and tolerability of each patient. The four types of insulin include rapid, short, intermediate, and long-acting preparations and mixtures of preparations with different onsets and durations. In addition, a broader range of insulin delivery systems such as with subcutaneous infusion pumps ; has made it easier to achieve tighter control of blood glucose.

Our study confirms and supports the earlier findings regarding usefulness of Chitosan as a wound-healing accelerator, and its effectiveness in protecting wound from bacterial invasion by suppressing bacterial proliferation. Our study further reveals that Chitosan may act effectively against typhoid-producing microorganisms. Moreover, we could establish that Chitosan shows antimicrobial activity; however, glucosamine does not show such activity. Antimicrobial susceptibility testing using discs showed that Chitosan is not effective against one of the resistant strains of S. enterica S. enterica-4 ; . However, it is quite effective against resistant strains of S. enterica S. enterica2 ; , S. enterica var. Paratyphi-A and S. enterica var. Paratyphi-B Figure 1 ; . Further investigations need to be done to develop Chitosan into a useful therapeutic tool for treatment of typhoid. Chitosan shows great potential in this respect, especially on the grounds of limitations such as development of resistance to side effects and toxicity of currently used antibiotics.
Tertiary amines such as amitriptyline, clomipramine and imipramine are much more likely to cause orthostatic hypotension than secondary amines such as desipramine, protriptyline and nortriptyline As a general rule, tertiary amines also tend to produce more sedation, presumably by blocking synaptic serotonin reuptake or by affecting histamine receptors. Anticholinergic and noradrenergic side effects are also particularly troublesome in both nondemented and demented elderly patients. Anticholinergic symptoms may be prominent both central delirium, confusion and cognitive impairment ; and peripheral tachycardia, urinary retention, constipation, blurred vision and dry mouth ; . Because of the well-documented deficits in the cholinergic system of demented patients and the effects of anticholinergic drugs on memory even in normal subjects, any potent anticholinergic medication given to a demented patient will be likely to worsen cognitive functioning even if it improves mood [7] and pepcid.

This limits use for people who do not have access to refrigeration, particularly in hot countries. Capparelli from University of California, Sand Diego and colleagues performed a small-scale proof of concept study in nonrecommended higher temperatures, to evaluate use where refrigeration is not available, particularly in reference to SubSaharan Africa. Kaletra capsules were incubated, individually and in a group of six, in high density polythene bottles, in high humidity, at 35 C and 45 C and contents tested for capsule stability and potency at day 1, 2 and 7 and week 2, 4 and 8. Physical structure was maintained at 35 for 4 weeks but capsules become distorted by week 8, increasing in weight by 3% and 11% respectively. At 45 degrees the capsules melted together within a week. Potency compared to storage at 4 was 95% at week 4 at both 35 C and 45 C but fell below 85% by week 8. The study concluded, that dispensing monthly supplies of LPV r in climates up to 35 where refrigeration is not available, would maintain potency, and use of this drug in wider settings than is currently available. It did not claim regulatory-standard assessment though, and highlighted the need for formulations that remain more stable at higher temperatures, for instance, southern orthopedics.
