Figure 2.1: Vector Space Model. Documents can be represented as vectors of word counts. Each dimension of the vector contains the number of times a word appears in the document. If a word does not occur in the document, the value for the corresponding dimension of the vector is 0. where wi is the number of times that word appears in the document and n is the number of unique words in the corpus. Since a typical document contains only small subset of all the possible words, these vectors are almost always sparse. Most of the values in the vector are zero. The VSM document representation is simple and discards all information derived from the ordering of the words. No information about context, phrases, modifiers, syntax or other structure is retained, leading some to call it a bag-ofwords model. Although the lack of structural information seems like an exigent deficiency, in practice, VSM performs acceptably well for many applications.

The demonstration centres which are also urban referral centres ; have a higher death rate than expected, even with the new protocols. During the busiest months of November through to March, death rates were between 30-40% in the main referral centres in north, central and southern regions. This may be due to the fact that complicated cases of malnutrition, many of which are exacerbated by HIV, are referred to the urban NRUs. In addition, these centres have poor levels of staffing, lack of clinical support, higher staff turnover, and are often over crowded in unhygienic conditions. There is also the problem of late presentation to the NRU. This is often due to a widespread belief in Malawi that malnutrition is a curse and not a condition to be treated medically. The common belief is that the children become.

A Brief History of the Directory Work on the New York State Directory of AIDS Clinical Trials began in 1988 when members of the AIDS activist group ACT UP NY became aware that AI DS clinical trials were not fully enrolled and were not adequately representative of all people living with HIV AIDS. People of color, women and substance users were virtually absent from clinical trials. To address this situation, a non-profit organization called the AIDS Treatment Registry ATR ; was formed to gather and disseminate information on clinical trials in New York State. ATR was also committed to advocating for better research priorities and better trial design. Over the years, the directory has undergone numerous changes in format, and various groups have taken on the task of producing it, including amfAR, ATDN, and now ACRIA. The directory is produced with support from the New York State Department of Health AI DS Institute.

1 Pneumococcal vaccine It is advised that pneumococcal vaccination should be repeated at intervals of 5 years. Haemophilus Influenzae Type B vaccination If Haemophilus influenzae type b vaccination has already been given as part of the childhood immunisation programme there is no need to repeat its administration The need for reimmunisation is unclear at present. Neisseria meningitidis groups A & C vaccination Debate continues to surround the scope of meningococcal vaccination. Some groups recommend it only for travel abroad Working Party of the British Committee for Standards in Haematology Task Force, 1996 ; whilst others have a more open view and believe it should be considered for all such patients Chief Medical Officer, 1994 ; . Until there is a consensus opinion we believe that it is appropriate to advise that the vaccine, which is very safe to administer, should be made available on the wider basis. We recommend at present that this should be a once only vaccination 4 Neisseria meningitidis group B There is no effective vaccine to protect against Neisseria meningitidis Group B which is the organism responsible for 70% of cases of meningococcal disease in Scotland and parlodel.

For your nipples, you will be prescribed an antifungal ointment or cream. Apply the ointment sparingly after each breastfeeding. Rub it onto your nipples and the area of the breasts that the baby's mouth covers. By the next feeding most of the medicine will have rubbed off on your clothing or breast pads, so there is no need to wash the nipples before breastfeeding.10 If there is a large amount of medicine left, then you may wish to wipe it off gently before feeding your baby. Perhaps olive oil on a cotton ball would be a good method to remove the ointment. If you are using breast pads, a fresh set should be used following each feeding. It is important to put on a clean bra every day. It has been reported that children have greater difficulties to recognize tastes in mixtures than adults due to their limited analytical skills in perceptual tasks. Their analytical ability evolves in pre-school years and increases until adolescence. However, their ability to recognize a flavour may also be affected by the concentration of the flavour in the formulation and the appearance of the medication itself. For example, a strawberry flavour-containing formulation was identified as chocolate because of its brown colour, indicating a strong association between colour and flavour. Whether children can analyse and recognize more than one flavour component in a taste mixture is unknown and the concentration of each of the flavours may affect a child's assessment. Enhancing the degree of recognition by avoiding unusual flavours and complex taste mixtures increases the probability that a formulation will be accepted by children. 5.2.2 Acceptance of the flavour by the target population and pioglitazone.

Figure 7 shows that scores on the SF-36 mental health subscale improved in all three groups, from around 37 at baseline to around 70 at 12-month follow-up, for the total sample of patients randomised, and when including only those who accepted the allocated treatment. Figure 8 shows that role limitation due to emotional problems as measured by the SF-36 improved from mean scores of around 23 to around 75, in all three groups over the 12 months of follow-up.

The use of anabolic steroids amongst adolescents is of great concern to the AOC. The Black Committee heard evidence of athletes as young as fourteen using steroids and it is clearly evident that young persons have access to, and use, anabolic steroids. Recent studies of anabolic steroid use in the United States indicate that the rate of use of anabolic steroids is between 4 and 11% of male adolescents, and between 0.5 to 2.9% of female adolescents. In circumstances where there are considerable adverse side effects of anabolic steroid use, some of which are irreversible, all necessary steps should be taken to ensure that adolescents cannot obtain and use these sports drugs.
RESEARCH Developmental regulation of transport and homeostatic processes in mammalian oocytes and preimplantation embryos My laboratory is interested in fertilization and early embryo development in the mammal, especially physiologically important cellular processes which are developmentally regulated. We are particularly interested in homeostatic and regulatory mechanisms which undergo major changes in activity or function during egg growth and maturation, fertilization, or preimplantation embryo development. One area of interest is the regulation of cell size in embryos. Animal cells control their volumes by regulating internal osmotic pressure. Embryos, like some other cell types, require small, neutral organic compounds to provide a portion of the intracellular osmotic support, and providing these organic osmolytes turns out to be a key factor in producing healthy embryos. Although early preimplantation mouse embryos use some of the same organic osmolytes as other cells, including glycine and glutamine, they lack expression of known mammalian organic osmolyte transporters which can transport these compounds. Instead, we have found the GLYT1 glycine transporter acts as an organic osmolyte transporter in embryos, a 15.

Emotional experience as the neutral ones. The pattern of neural activity for both the healthy participants and the obsessive compulsive disorder patients was similar, with the aversive scenes activating the insula. However, the aversive scenes did not activate this region in the depersonalised patients. Instead a greater neural response more activity ; to the neutral scenes was demonstrated in these areas Phillips et al., 2000 ; . A final study, currently in progress, aims to build on the finding described above by examining the neural processes underlying emotional memory both in healthy participants and in those with depersonalisation. Emotional memory has become a key area in affective neuroscience, due in part to its relevance in a range of psychiatric disorders including post-traumatic stress disorder, depression, and phobias. We are currently using stimuli specifically designed to examine the recall of emotional and neutral information from affective contexts. A previous pilot study carried out on healthy participants has shown a differential neural response to, on the one hand, direct emotional recall recalling an emotional word, e.g. `raped' ; and, on the other hand, contextual recall recalling the sentence the word was read in, e.g. `the sailor raped the girl in the alley' ; Medford et al., 2000 ; . We are currently using the same technique to examine these neural processes in the context of depersonalisation. Using the internet as a research tool Recent psychological investigations carried out on the internet have replicated results from standard laboratory and pen-andpaper studies for reviews see Senior et al., 1999; Senior & Smith, 1999 ; . These studies suggest that the internet is a viable medium for psychological research, although its role in clinical research has only just begun to be explored see Senior et al., 1997; Smith & Senior, 2001 ; . We wanted to assess the utility of the internet for clinical research into depersonalisation. To do this we compared the scores of several clinical and demographic questionnaires between participants recruited on the depersonalisation bulletin board see Weblinks ; and the unit's outpatient clinic. We predicted that the general demographics and clinical psychometric scores between the two groups would be the same. All participants were asked to provide details of their history of depersonalisation, for example, nuvaring and monistat.
